You set your back under a mired wagon and lifted with everything you own. Low in your belly, something is deciding whether to hold.

Strangled in the Ring
CatastrophicYou felt something tear low and hot as the wagon came up, and by nightfall the bulge in your groin has gone hard as a fist, dusky purple, and you are vomiting bile. The gut is caught in the tear and dying there. The doctor lays you out under chloroform and opens the ring with a knife by lamplight, carbolic and silk, racing the rot inside. If the loop is black when he reaches it he must cut the dead length away and pray the join holds, and men do not always wake from that.
- Bedridden after the knife, no standing unaided for 5 days, nothing heavier than a cup lifted for 6 weeks
- Belly bound over the incision, sutures checked daily for fever, redness and seeping
- Broth and water only until the gut moves again, 3 days at the least
- /me lies gray and still beneath the blanket, knees drawn up over the bound groin
Recovery Six to eight weeks if fever spares you, and the wall stays weak for a season after.·Doctor, urgently
2
Caught and Pinched
SevereThe bulge came out at the lift and would not go home. By the time the doctor sees you it is tight and tender, the gut pinched in the tear and swelling by the hour. He lays you on a board with your hips raised, dopes you with laudanum until your belly softens, and works the loop back through with slow kneading fingers. It goes at last, with a gurgle you will remember, and he tells you flat that the next time it may not.
- Strict bed rest 3 days, hips kept raised on a folded blanket
- Fitted with a steel and leather truss, worn every waking hour for 6 weeks
- No lifting, straining or hard coughing for 6 weeks; if the bulge returns hard and will not go back, a doctor at once
- /me keeps one hand pressed flat over the trussed groin when rising
Recovery Three days abed, six weeks trussed and idle before the wall can be trusted at all.·Doctor, urgently
3
The Wall Gave Way
SevereYou heard nothing, only felt the muscle tear like wet canvas low on the right side, and the weight of your own insides sagging into the gap. The bulge is broad as a palm. It goes back when you lie flat, then falls out again the moment you stand. The doctor binds you into a truss and gives you the choice laboring men hate: weeks flat on your back now, or the surgeon's silk in the city, or a strangled gut later.
- Bedridden or reclined for 10 days, on your feet only for necessities
- Truss and belly binding worn day and night for 8 weeks
- No lifting above 10 pounds for 2 months, no riding above a walk for 3 weeks
- /me lowers into the chair in stages, one forearm braced along the beltline
Recovery Ten days down, two months trussed, and the tear never truly knits without a surgeon.·Doctor required
4
Torn and Retching
SevereThe tear came with a heat like a branding iron laid inside your groin, and your stomach turned itself out on your boots before the crate hit the ground. There is a bulge, small but sullen, and every retch drives it harder against the torn ring. The doctor works it back, doses you with laudanum to quiet the heaving, and forbids you anything more strenuous than turning over in bed.
- Abed 5 days, laudanum at night, sips of broth and water only the first day
- Truss worn 6 weeks once the swelling settles, no lifting all that while
- Every cough and sneeze splinted with both hands over the groin for 2 weeks
- /me turns onto one side in slow stages, both hands cupped low over the belly
Recovery Five days flat, six weeks trussed, and the ring stays tender past two months.·Doctor required
5
The Bulge That Stays
SeriousSomething rolled out low on your right side when the axle came free, a lump like a goose egg that aches with a dragging, seasick weight. It slips back when you lie down and press it, and slips out again when you so much as clear your throat. The doctor calls it a rupture, fits you for a truss from the mercantile, and gives you the truth with the bill: no lifting for weeks, or the truss becomes the least of your troubles.
- Truss worn every waking hour for 6 weeks
- No lifting above 20 pounds, no pushing wagons, no hauling on ropes for 6 weeks
- Dismount by stepping down easy, no vaulting from the saddle for a month
- /me presses the heel of a hand low against the beltline before bending down
Recovery Six weeks trussed and careful; the weakness in the wall outlasts the ache.·Doctor required
6
Burst at the Navel
SeriousThe strain found the old weak spot every laboring man carries, and your navel now stands out like a knuckle, tender and wrong. The doctor kneads it flat while you lie with your knees up, then straps a coin-hard pad over it and binds your middle like a barrel. Umbilical rupture, he says, common as bad teeth, and just as likely to turn cruel if you ignore it.
- Belly binding with a hard pad over the navel, worn day and night for 5 weeks
- No lifting above 15 pounds and no straining at the jakes for 5 weeks, meals kept soft and light
- /me hooks a thumb under the binding at the waist and resettles the pad
Recovery Five weeks bound; the navel stays proud and tender for a season.·Doctor required
7
A Dragging Weight
SeriousNo sharp tear, just a slow giving way, and now there is a heaviness low in your groin like a stone in a feed sack, worse by evening, eased by lying flat. The doctor finds the ring widened and the gut leaning into it. Not through yet, he says, but leaning. He binds you, forbids the heavy work that brought you here, and promises you a true rupture within the month if you defy him.
- Binding worn through the workday for 4 weeks
- No lifting above 25 pounds, no long days in the saddle for 4 weeks
- Lie flat with knees bent for a quarter hour, morning and night, for 2 weeks
- /me shifts weight off one hip and straightens by careful degrees
Recovery Four weeks of light duty; the dragging ache fades inside two if you behave.·Doctor required
8
Split Along the Seam
SeriousThe muscle along your flank tore like a rotted seam when the crate tilted, a strip of fire from hip bone to short ribs. No rupture, the doctor says after a long prodding examination that leaves you sweating, but the wall there is torn and bruising deep, and it is the wall that keeps your insides where God put them. He binds your trunk tight and rations your labor like a mean quartermaster.
- Trunk bound from hips to ribs for 3 weeks
- No lifting above 20 pounds, no swinging an axe or hammer for 3 weeks
- Sleeping on the sound side only for 10 days
- /me straightens off the wagon wheel slowly, one hand flat along the flank
Recovery Three weeks bound and idle, a yellow-green bruise blooming for two of them.·Doctor advised
9
The Cough That Stabs
ModerateThe lift went through, the wagon rolled, and you thought you were clear until the first cough dropped you to a knee. Something low in your belly stabs like a hat pin with every cough, sneeze and laugh. The doctor probes the groin ring, finds it sound but strained, and tells you that the difference between you and a ruptured man is about one more foolish lift.
- No lifting above 30 pounds for 2 weeks
- A hand pressed hard over the groin before every cough or sneeze for 10 days
- /me stiffens mid-laugh and flattens a palm low against the belly
Recovery The stab dulls in a week, gone in two if the lifting waits.·Doctor advised
10
Bent at the Waist
ModerateYou set the crate down wrong and your groin lit up like a struck match. Now you walk with a hitch, folded an inch forward at the waist, because standing tall pulls at something down there that does not want pulling. A strain of the deep groin, mean but honest, and the wall behind it is whole.
- Walking bent and slow for 4 days, no long strides
- No mounting a horse from the ground for 5 days, use a fence rail or a wagon step
- Warm cloths laid on the groin each evening for 3 days
- /me walks folded a shade forward at the waist, steps short and even
Recovery Straightens up over four or five days, sound inside a fortnight.·Doctor advised
11
Strained to the Spine
ModerateThe load took your belly and your back both. Something pulled low in front and something seized behind, and now the two argue over which of them owns you. The small of your back is locked in a board-stiff clench, and the belly complains whenever the back lets it get a word in.
- No lifting for 10 days, no shoveling, no chopping wood
- Hard chairs only, rising by pushing off the knees, for 1 week
- Liniment rubbed into the lower back morning and night for 5 days
- /me rises from the chair in one careful piece, hands pushing off the knees
Recovery The back frees up inside a week, the belly a few days after.·Doctor advised
12
It Went Back With a Push
ModerateFor one bad minute there was a lump low on your belly the size of a robin's egg, and then you lay flat in the wagon bed, pushed with two fingers, and it slid home with a soft click you felt more than heard. The doctor finds the ring loose but the wall serviceable. A beginning rupture, he says. Whether it finishes is up to you and what you refuse to lift.
- A snug binding worn low on the belly through work hours for 3 weeks
- No lifting above 25 pounds for 3 weeks, and lift with the legs ever after
- /me tightens the low binding a notch before taking up any load
Recovery Three careful weeks; the ring tightens if it is left in peace.·Doctor advised
13
Winded at the Wheel
ModerateThe wagon settled back into the mire and took your wind with it. You strained everything a man can strain from thighs to throat, and for a long moment you stood gripping the wheel and seeing sparks. It is all muscle, every bit of it, and every bit of it means to remind you for days.
- Sore from forearms to thighs, half-speed labor for 3 days
- No wind for running or brawling for 2 days
- /me flexes both hands slowly, wincing from wrists to waist
Recovery Three days of deep soreness, gone within the week.·No doctor needed
14
A Low Pull
MinorA pull, low and to the left, sharp for a heartbeat and dull ever since. You finished the lift and you paid the pinch for it. There is nothing to see and nothing to bind, only a muscle in your lower belly that objects to sudden movement and second helpings.
- Sore lower belly, no heavy lifting for 4 days
- Rise from bed by rolling to the side first for 3 days
- /me rolls to one side before sitting up, unhurried about it
Recovery Aches four days, forgotten inside a week.·No doctor needed
15
Groin Gone Tender
MinorYou caught the weight wrong and your groin muscle took the bill. Walking is fine, stairs are rude, and swinging a leg over a saddle is a short prayer. It is a strain and nothing more, though it makes certain you know it is there.
- Ginger in the saddle for 3 days, mounting from a step where one can be had
- No running or jumping ditches for 4 days
- /me swings a leg over the saddle slow, letting the horse stand for it
Recovery Tender three or four days, sound in a week.·No doctor needed
16
A Twinge and a Warning
MinorSomething twinged low in your belly at the top of the lift, sharp enough to make you set the crate down like it was full of eggs. It has faded to a whisper already, but it was your body reading you the warrant: the wall down there is tired, and the next lift like that one gets charged full price.
- No solo lifting of anything above 40 pounds for 3 days
- A twinge low in the belly when reaching overhead for 2 days
- /me sets the crate down slow and square, knees bent, back straight
Recovery A whisper by tomorrow, gone in three days.·No doctor needed
17
Lift With the Legs
MinorYou felt the strain gather low and mean, and some old ghost of good sense made you drop your hips and take the weight with your thighs at the last second. Your legs burn like you climbed a mountain and your belly aches only faintly, a cheap fine for a lesson most men pay for in trusses.
- Thighs stiff as fence posts for 2 days, stairs taken one at a time
- A faint low belly ache through tomorrow
- /me eases down the porch steps one at a time, thighs complaining
Recovery Two days of stiff legs, and the belly forgets by morning.·No doctor needed
18
Only the Belt Gave
LuckyThere was a crack and a give at the worst possible instant, and your heart dropped through your boots, and then your trousers nearly followed it. Your belt snapped clean at the buckle hole. The lift was lost, the wagon settled back easy, and you stand there holding your waistband with one hand and your dignity with neither.
- A red welt across the waist where the belt bit before it broke, tender for a day
- In want of a belt or a length of rope until the mercantile opens
- /me hitches the waistband up with one fist and surveys the snapped belt
Recovery The welt fades by tomorrow; the belt is past praying for.·No doctor needed
19
The Load Shifted
LuckyAt the last breath, at the very top of the strain, the load shifted and rolled its weight onto the wheel and off your spine, and the wagon came free like it had only been teasing. You felt the danger pass low in your belly, a flutter where a tear was choosing not to happen. Your hands took the worst of it, and the worst is a rope burn.
- A rope-burned right palm, gloves or a rag wrap for 2 days
- A healthy new respect for mired wagons and green ore crates
- /me flexes the rope-burned palm open and shut, whistling low
Recovery The palm smarts a day or two, and that is the whole of the bill.·No doctor needed

The Lift They Still Talk About
MiraculousThe wagon should have ruptured you. Every man watching swears to it. But when you set your back and heaved, the axle groaned up out of the mud in one impossible rise, the mire let go with a sound like a boot pulled from a bog, and the whole load rolled free while you stood up straight and whole. Somewhere in the middle of it the mud took your boot instead of your belly taking the tear. The story is already growing legs at the bunkhouse, and by Sunday you will have lifted the wagon, the ore and the mule besides.
- One boot lost to the mire, walking odd until it is dug out or replaced
- Whole and unhurt, though every hand at camp wants the lift retold nightly
- /me rolls both shoulders loose and grins at the wheel ruts, hands on hips
Recovery Nothing to heal; the tale grows a pound heavier with every telling.·No doctor needed
